You must complete and submit a Claim for Damages form and include documentation of estimates, receipts, and/or photos in support of the claim.

The City will investigate the claim with the related department or staff member and forward all information to our insurance authority who will examine the claim, may use the services of an adjuster, and upon completion of the investigation, will make the final determination if negligence is found.
